THE FOLLOWING IS THE INFORMATION ON THIS COURSE:

COURSE COST: $1,485.00

COURSE DURATION/SIZE: 4.5 days, min. 40 hrs of lecture and practical exercises, plus 2 hr written exam. Class size is limited to four to twenty students per session.

COURSE MATERIALS: Each student shall be provided with a complete set of course notes to supplement the training program and a copy of ISO9001 Standard at the beginning of the course. Students shall have a general knowledge of the Standard's requirements prior to the course. A short quiz is conducted on day 1 of the course. No audio recording or video taping is allowed.

ATTENDANCE: Students are required to be in attendance at each class session. Due to an emergency, or illness, students should call our office to coordinate a make-up class (Four hours maximum - upon approval).

LODGING: The information on the course location is provided to all students at least 20 days in advance. Students are encouraged to stay at the same facility, or as close as possible to the course location.

RIGHT TO APPEAL: Upon request, the instructor will provide the information about the right to appeal. It this information is not adequate, please contact our Chicago Office immediately.

METHOD OF PAYMENT AND CANCELLATION (APPLIES TO ALL COURSES OFFERED BY AQE GROUP): All payments are due prior to the beginning of day 1 of the course. Purchase Orders, when accepted, are treated the same as a full payment, non-cancelable. Under no condition will any cancellation be accepted after the beginning of the class. Visa, Master Card, American Express, Discover, Money Orders, Certified Checks, Company Checks and Personal Checks are accepted as a form of payment. A $25.00 service fee applies to all returned checks. Registered and paid participants who fail to attend the class are liable for the entire tuition. A $250 cancellation fee will be applied for cancellations received less than 10 days prior to the start of the course. Otherwise, payments are non-refundable.
